Minor Waiver Form

Acknowledgement and Assumption of Risk

I, the parent/guardian specified in the attached application form, acknowledge that I have voluntarily applied for the Immeasurable Journeys trip on behalf of the minor specified in the attached application form. I understand that he/she may travel to remote areas that are inherently risky and that may include activities associated with foreign travel. I am prepared to assume the risks associated with this trip including: forces of nature, terrorism, civil unrest, war, accidents and transportation including land vehicles, boats, and aircraft that are not operated or maintained to standards found in North America.

I also assume risks associated with altitude, illness, disease, physical exertion and knowing that access to evacuation and/or suitable medical supplies and support may not be immediately available. By signing this document,
I agree to take full responsibility for the minors actions, safety and welfare, including but not limited to unanticipated events including injury, illness, emotional trauma, or death. I also understand that he/she will be a member of a group and will conduct himself/herself in a way that will not endanger the group or myself, in accordance with the Trip Code of Conduct set out by Immeasurable Journeys. I understand that if I fraudulently represent him/her for this trip, he/she may be removed prior to or during the trip at my own expense.

Agreements of Release and Indemnity

I, hereby acknowledge that the aforementioned minor is presently under my care, custody and control. I hereby give this child my express permission to go on the above Immeasurable Journeys trip. In the event of an emergency, necessitating surgical attention, I hereby consent and give my permission to the staff of Immeasurable Journeys or any attending physician to make necessary decisions and perform medical treatments and/or surgery upon the aforementioned minor which may be necessary and proper under the circumstances.

I do release, acquit, discharge and covenant to hold harmless the Immeasurable Journeys staff from any and all actions, damages and liabilities arising out of the treatment of any sickness or incident, incurred by my said child during the above marked dates of the Immeasurable Journeys trip. In doing so I waive any and all claims that I now have or may have in the future against Immeasurable Journeys. I agree that this release will be legally binding upon the child, myself, my heirs, successors, assigns and legal representatives; it being my intention to fully assume all risk of travel of said minor and to release Immeasurable Journeys from any and all liabilities to the maximum permitted by law.
